Can't get past customization of a game

I can't quite get past customization of a game I bought to try out (Sexual Sacrifice 2 Tentacles and Lust Beast Island). I know that one of the buttons above (F4) closes the game and (F1) to get to instructions. I'm very confused (I've tried pressing everything)
