It's a nice idea I am planning to add, but it may increase complexity. Thats because Peach has three ways to maintain her life before losing it:
1) Some Powerups: when she's using Tanooki, getting harm makes her lose these powerups instantly, like Mario 3. But other powerups let her lose a clothing item.
2) When she loses the last piece of clothing, her powerup loses its function (like frog , leaf and fire) and she becames naked.
3) Finally, further harm will let her lose horny bar amounts. If horny bar is too small, finally she will lose a life. This was done on purpose, to let the game easier, as the main objective is not to create difficulty, but to let the player enjoy the scenes.
So, in order for the backup powerup falling to work correctly,
it should be activated no matter the 1,2,3 above. Every harm will trigger backup powerup. I think its better to activate backup powerup only if she is stark naked.
I've thought of changing big mushroom mechanics, so she would be with maximum breasts size and mushroom icon would appear in the HUD area, but this will impact different breast size amounts that gets activated when taking future mushrooms. Also, some puzzles will be non-sense (e.g. when peach refuses, Breasts maxes out too).
But I'm brainstorming of letting Powerups disappear after getting a single harm, no matter if Peach is with all clothing items or not, like in Fire Dress. She will still be able to lose a cloth item while fucking, but she will not lose the powerup this way. Only when getting harmed. A voting pool could give more light.

Peach being Little Peach doesnt work well because I'll have to re-create her art, as just resizing her may look awkward. Mario and Super Mario are different models. Mario uses to have a bigger head in regard to its little size. Also, sex scenes will not work in Little Peach. Only if I create all of them from scratch (sex scenes specific for Little Peach).
Mini Peach, however, is a possible addon, but sex scenes will not be activated. Peach will get harm instead. Peach could be a mini Peach to go to certain areas or to flee. And it may have a timer (1 minute).

The checkpoints already clean her cum. I intend to put small bath houses along the level, like LoK. So she will take a bath.
I could use the yellow/white leaf (like New Super Mario Bros 2) for an aphrodisiac. Her horny bar could increase until the top and she may moan and walk awkward. But I prefer to do priorities first.

I dont intend to do that because, at first, Peach is a bit innocent in this game (she prefer to stay away of sex). Second, this will stay so away of classic Mario, and it will be much more porn oriented (like the Hentai movie StarBallz). I prefer normal fireballs (when she gots fire powerup) which will kick him instead.
