Glitcherino is
Missingno FA's form. I can't find anything solid on its appearance, other than words, saying that it starts out looking like a glitched Nidorino, but "fixes" itself once its HP fixes. What little I'm seeing, it looks like it has no eyes, is missing its horn, and its ears are rounded instead of pointed.
Missingno as a trainer might be something. There are several sprites associated with Missingno's. There's the backwards L, the 190º rotated backwards L (r), the square, broken Bulbasaur (which I think it work best playing the fact that its a Hybrid of Dodrio, make a Bulbadrio or something), Glitcherino, and Bars. You could fit the ghost right in as the sixth, and leave the skeletons to Krystal. Since the proper slots for pokemon in the game is actually five, since the trainer is counted as the first of six Pokaymon, you can't just give her six and call it fair. Kabutops/Aerodactyl skeletons, Mew, and then she'd have two more slots available. If you throw in the ghost, since her Trainer name would allow her to catch it, that leaves one more slot. My opinion on this last slot is that Dark/Clone Pikachu (the one made by Mewtwo) or Pink Butterfree. There aren't a whole lot of "extras" that were made in Gen 1, but those two definitely stand out.

Missingno. called TRAINER. It basically appears as a Magnemite.
